Leaving hospital can feel overwhelming. Our Bridging Service is here to make that step smoother and safer. We provide tailored support for adults as they move from hospital care back into the community, ensuring no one feels alone during the transition.

What we offer includes:

A safe, welcoming environment on discharge

Practical help with settling back into daily routines

Guidance in accessing local services, GPs, and community resources

Emotional support and reassurance during a time of change

Pathways to independence through life skills and community connection

Additional Services Tailored to Mental Well-being and Recovery

1. Peer Support Groups
Small, friendly groups where people can share experiences, encourage each other, and reduce isolation.

2. Wellbeing Workshops
Practical sessions on topics like stress management, healthy routines, sleep hygiene, mindfulness, or creative outlets (art, journaling, gardening).

3. Befriending / Buddy Schemes
Volunteers matched with individuals for regular check-ins, walks, or social activities — helping rebuild trust and connection.

4. Supported Volunteering & Community Activities
Opportunities to get involved in safe, structured volunteering roles or group projects that give purpose and build confidence.

5. Digital & Daily Living Skills
Helping people learn to use smartphones, online services, budgeting apps, or community resources so they can feel more independent.

6. Meaningful Activities Programme
Group or 1:1 activities like cooking clubs, walking groups, gardening, or arts and crafts that promote wellbeing and routine.

7. Advocacy & Navigation Support
Guidance in understanding benefits, housing, or other systems that can be stressful post-discharge — without providing formal legal/clinical advice.
